Overview

Klein's interview with Ramaswamy was released on October 29, 2024, after Ramaswamy's run for the 2024 Republican presidential nomination and before he entered the race for governor. Klein asked whether Medicare and Medicaid were mistakes. Ramaswamy answered that he believed they were, 'with the benefit of retrospect,' and singled out Medicaid and the welfare state without work requirements. He then said Medicare and Social Security are in a different category, and argued that the welfare state has harmed the people it was created to help and that the benefits created in the 1960s will eventually run out in a national debt crisis.

MeidasTouch posted the exchange on January 16, 2026. The Democratic Governors Association circulated it, and its spokesperson Izzi Levy said Ramaswamy 'didn't even need to think twice before endorsing ripping away health care from millions of Ohioans.' Acton wrote on social media that a billionaire who flies around Ohio on a private jet had called these programs a mistake.

In February, the Statehouse News Bureau reported that Ramaswamy's campaign called Acton's attacks blatantly dishonest and based on 'a cherry-picked 30 seconds from an 80-minute interview,' and that in a written statement the campaign said 'Medicaid is a vital program that millions of Ohioans rely on.' In October, after the Ohio State Medical Association's political action committee endorsed Ramaswamy, more than a thousand doctors signed a letter backing Acton; the Statehouse News Bureau reported that they are concerned he could limit access to the programs because of the remark, and that Ramaswamy has said the comments were taken out of context. On October 23, Acton posted that Ramaswamy calls Medicare and Medicaid mistakes and said she would protect both programs.

Ramaswamy, a biotech entrepreneur who sought the 2024 Republican presidential nomination and worked briefly on the Department of Government Efficiency effort with Elon Musk in early 2025, is the Republican nominee for Ohio governor. Acton, a physician and former director of the Ohio Department of Health, is the Democratic nominee. The election is November 3, 2026.

Medicaid work requirements are already federal law: the tax and spending bill President Trump signed in July 2025 requires most recipients between 19 and 64 to log 80 hours a month of work, training or community service, with compliance due by January 1, 2027. Both candidates have voiced support for work requirements in some form.
