Presenting the military's inquiry into the battle for Kibbutz Be'eri, IDF spokesperson Rear Adm. Daniel Hagari said the police, Shin Bet and army personnel at Pessi Cohen's house "operated professionally and orderly" and fired tank shells only after negotiations were exhausted and gunfire from inside raised fears that hostages were being executed. He said the inquiry found that, to the best of its assessment, no civilians inside the house were hit by tank fire, and that it is highly likely one hostage outside was killed by tank shrapnel.

The IDF said its inquiry found that, to the best of its assessment, no civilians inside Pessi Cohen's house in Kibbutz Be'eri were hit by tank fire, that most of the hostages inside were apparently killed by the attackers, and that one hostage outside the house was highly likely killed by tank shrapnel.
“The inquiry team determined that under the complex and harsh circumstances, the Yamam, ISA, and IDF personnel operated professionally and orderly, making professional and responsible decisions. They exhausted all negotiation efforts with the terrorists. Only after the situation deteriorated and negotiation attempts were exhausted – attempts to negotiate the rescue of the civilians – did the forces carry out tank fire, in a joint decision to try to apply pressure to the terrorists. The decision to attempt a rescue under fire was made after all negotiation attempts to rescue the hostages had been exhausted, and after a long burst of gunfire from inside the house that raised suspicions that the hostages were being executed. We conducted a thorough and joint inquiry with the Israel Police, Yamam, and the ISA, after which we presented our assessment of the circumstances of the hostages' deaths to the families. The team determined that from the information examined so far and to the best of our assessment, no civilians inside the building were hit by tank fire. The team noted that it appears that most of the hostages inside the house were murdered by the terrorists. Additionally, one incident involved hostages outside the building being hit by tank shrapnel, and it is highly likely that one of them was killed as a result.”

On October 7, 2023, Hamas-led attackers struck southern Israel. About 1,200 people were killed in Israel that day and about 250 were taken hostage into Gaza. In July 2024, Haaretz reported that documents and testimony show the Israeli military used the Hannibal procedure, which directs force to stop soldiers from being taken captive, at three army sites that day. The detailed findings of a UN commission of inquiry say Israeli fire likely killed at least 14 Israeli civilians in two incidents. The IDF has said a helicopter likely killed Efrat Katz of Kibbutz Nir Oz and that tank shrapnel highly likely killed one hostage outside a house in Kibbutz Be'eri, and it has defended the decisions of its commanders. Former defense minister Yoav Gallant said in February 2025 that he thinks an order to apply the procedure was given tactically in some places and not in others. The dispute is over whether, and how often, Israeli forces fired on Israeli captives that day, and how many Israelis that fire killed.

It says the commission documented strong indications that the Hannibal Directive was used in several instances on October 7, cites a tank commander's statement to an Israeli news channel that his crew targeted two vehicles carrying militants and Israelis near Kibbutz Nir Oz, and concludes that in two cases at least 14 Israeli civilians were likely killed by Israeli fire: one woman by helicopter fire as she was taken from Nir Oz, and 13 by tank shelling and crossfire in Kibbutz Be'eri. The commission says it could not confirm whether helicopters fired at targets in the Nova festival area. The commission says Israeli officials refused to cooperate with its investigation.
